Helping Residents Achieve a Healthy Lifestyle
In developing the 2025-2028 Community Health Improvement Plan (CHIP), the GMPHP adopted overarching goals and objectives to address health equity. The graphic shows that healthcare entities and their partners can broadly impact health needs by addressing socioeconomic factors.
This approach does not discount public health campaigns and individualized care. These interventions play a significant role in individual health outcomes. Combining targeted interventions with community-wide strategies that address systematic inequities has the greatest potential to impact overall community health and well-being.
Using the objectives outlined below, GMPHP will use the 2025-2028 CHIP to monitor county-wide progress toward the priority areas. GMPHP partners will identify existing and future institutional strategies that align with the identified goals and share progress regarding their specific actions that work towards achieving the objectives through collective impact. 1) Mental & Behavioral Health - Mercer County nourishes the mental health and wellness of all, promoting a healthier community.
2) Access to Wellness - All in Mercer County have equitable access to physical, social, and economic resources to achieve their own optimal health and wellness.
3) Housing & Built Environment - Everyone in Mercer County has a place to feel safe and call home in a thriving community.
4) Maternal & Infant Health - All birthing people and infants in Mercer County have equitable access to quality care and resources across the continuum to improve health outcomes.
